Looking at the kernel logs it looks like a Storage issue.

I can see SCSI commands are constantly failing on few luns with sense code>H:0x0 D:0x2 P:0x0 Valid sense data: 0x5 0x24 0x0.
Eg:Lun>naa.600508b1001cd10a455c9eee86ca4c83
Sense code H:0x0 D:0x2 P:0x0 Valid sense data: 0x5 0x24 0x0.decodes to >This status is returned when there is no error on the host side. This is when you will see if there is a status for a Device or Plugin. It is also when you will see Valid sense data instead of Possible sense Data.
You have to check with your Storage Vendor to know why lun's are not responding/Command failures are happening.There will be certainly events/errors logged against the same in the Storage.Any questions? Feel free to revert
